311 Outreach Case Manager-SV/ Santa Monica Corps-11-160

The Outreach Case Manager works under the direction and supervision of the Outreach Coordinator. He/she performs outreach, information/referral, and case management. The Outreach Case Manager collaborates closely with other internal/external resources to provide resources that may be used to assist individuals in accessing services to include, but not limited to: food, shelter, clothing, legitimate income and permanent housing. Additionally, the Outreach Case Manager works with outreach team in assisting clients in navigating healthcare systems in order to access physical and mental health benefits and services. He/she updates and maintains client documentation and information in applicable computer software.
