About the Role

Support patients throughout their behavioral health journey by keeping care organized, accessible, and connected. As a Behavioral Health Care Coordinator, you'll serve as an important link between patients, families, Behavioral Health Care Managers, providers, and referral sources while supporting a primarily geriatric population. You'll coordinate referrals and appointments, educate patients and decision-makers about the Behavioral Health Program, and help ensure each individual understands what to expect as they begin and continue receiving care.

This role is ideal for an organized, compassionate professional who enjoys helping others, solving problems, and coordinating multiple priorities with limited supervision. You'll schedule initial and follow-up visits, coordinate IDG and telehealth sessions, obtain and maintain required consents, manage the behavioral health care registry, and respond to patient and family needs through clear and timely communication. Through your attention to detail, service-focused approach, and commitment to behavioral health and wellness, you'll help create a seamless care experience that keeps patients connected to the support and resources they need.

Location

  • Office Location: Jackson, MI

Hours

  • Monday to Friday, 8 a.m. to 5 p.m.

Required Qualifications

  • High School Graduate or GED
  • Minimum CNA or MA credentialed or equivalent work experience
  • Minimum 1 year experience in medical field, medical office, or related

Preferred Qualifications

  • Associate's degree
  • Experience with geriatric population with a concentration in behavioral health

Essential Skills and Competencies

  • Ability to operate computer equipment, fax and photocopy machines necessary
  • Must be proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
  • Excellent verbal and written communication skills
  • Must be able to communicate professionally with all levels within and outside of the organization
  • Strong grasp of medical terminology
Pay Range
$18—$22 USD
