CASEWORKER

The case worker participates as a member of the health care team by contributing to the understanding of the social, emotional and economic factors which relate to the patient's illness, disability and age. Provides counseling, crisis management, bereavement services, and arranges for home care, utilizing available community resources. Arranges for extended care facility placement as indicated. Communicates with all members of the interdisciplinary team and community agencies to facilite care within the continuum. Focurses on timely, appropriate discharge plans, coordinates with the case management nurses to ensure benefits for services and insurance authorization.
