Rehab at Home Physiotherapists
Our Rehab at Home program combines in-home physiotherapy, virtual care and multidisciplinary coordination to deliver exceptional rehabilitation outcomes.
You'll support patients recovering from elective orthopaedic surgery and acute hospital admissions, helping them achieve faster recovery, greater independence and improved quality of life in the comfort of their own homes.
Key Responsibilities
Deliver comprehensive physiotherapy assessments within patients' homes.
Develop and implement evidence-based rehabilitation programs.
Support patients recovering from orthopaedic surgery and acute hospital admissions.
Promote mobility, function and independence.
Utilise virtual care and digital health technologies.
Collaborate with medical, nursing and allied health professionals.
Support early discharge and hospital avoidance initiatives.
Maintain high-quality clinical documentation.
Contribute to service development and quality improvement activities.
About You
Current AHPRA registration as a Physiotherapist.
Minimum three (3) years' post-graduate physiotherapy experience.
Experience in rehabilitation, orthopaedics, subacute care or community-based services highly regarded.
Strong clinical reasoning and evidence-based decision-making skills.
Confidence working autonomously in community settings.
Current Driver's Licence and access to a reliable motor vehicle.
Hospital in the Home (HITH) Clinical Nurses
Our Hospital in the Home (HITH) service enables patients who would otherwise require hospital admission to safely receive acute-equivalent treatment in their own homes.
Working within a highly collaborative multidisciplinary team, you'll deliver advanced clinical care while supporting hospital avoidance, earlier discharge and improved patient experiences.
Key Responsibilities
Conduct comprehensive patient assessments and clinical reviews.
Develop and implement individualised care plans.
Deliver advanced nursing interventions, including IV therapies and complex wound care.
Monitor patients with acute and complex healthcare needs.
Coordinate referrals, admissions, care transitions and discharge planning.
Collaborate closely with medical, allied health and community providers.
Support patients, families and carers through education and care planning.
Participate in quality improvement and service development initiatives.
Provide mentorship and clinical support to team members where appropriate.
About You
Current AHPRA registration as a Registered Nurse.
Minimum three (3) years' post-graduate nursing experience.
Experience in acute medical, surgical, emergency, ICU, critical care, community nursing or HITH environments.
Demonstrated experience managing patients with complex clinical needs.
Strong clinical assessment and care coordination skills.
Confidence working independently within community settings.
Current Driver's Licence and full Australian working rights.
