Community Care Coordinator
Permanent, Full Time
Bristol, Hybrid - 2 days WFH every week
Between 7am & 7pm - hour hour shifts. 1 Weekend worked in every 3, and Bank Holidays on a Shift Basis

We are looking for a proactive and compassionate person to join our team and provide an excellent care experience by helping to ensure the quality and timely coordination of ongoing complex care packages.

đź’ˇ About the Role

In this role, you will:

  • Act as the initial point of contact for clients, candidates and service users by fulfilling routine and complex care requests.

  • Arranging the very best healthcare professionals to deliver quality care as per our client's health and social care needs

  • Take ownership of rotas and accountability of filling available shifts.

  • Ensure CQC compliance and administration for care packages is relevant and up to date

  • Ongoing attendance at weekly assessment meetings to ensure effective management and execution of quality care

  • be responsible for ongoing planning, monitoring and reporting on packages of care

This is a role where your ability to build relationships, attention to detail and curiosity will really make a difference.

  • Strong and demonstrable customer service experience.

  • Excellent communication skills with the ability to work well as part of a team.

  • High attention to detail.

  • Ability to manage a high volume of calls and remain calm under pressure.

  • Proactive approach to completing tasks
