Responsibilities

As a member of the senior leadership team, this role provides strategic leadership and oversight for the development, implementation, and continuous monitoring of quality improvement, patient safety, risk management and emergency management programs and serves as the subject matter expert for clinical documentation, Epic workflows, regulatory compliance, and staff education across all Dartmouth Health Home Care programs.  The position champions a culture of quality and safety through collaboration, role modeling, and education and establishes a systematic approach to identifying, measuring, prioritizing, and monitoring quality initiatives.  In addition, through collaboration with clinicians, clinical leaders and IT, this position provides ongoing support, assessment and identifications of technology needs. Serving as the organization’s Compliance Program Manager and Clinical Informaticist, the role partners with executive leadership, the Board of Trustees’ Corporate Compliance Committee, the Dartmouth Health Compliance Officer and the Dartmouth Health IT team to oversee compliance and ethics efforts, ensure regulatory adherence, drive effective program outcomes aligned with organizational mission, values, and strategic priorities and support Epic enhancements and clinical documentation optimization.

 

  • Lead the development, implementation, and annual evaluation of the organization-wide Quality Improvement (QI) Program in compliance with CMS Conditions of Participation. Design and implement performance improvement and corrective action plans, ensuring integration of evidence-based practices into home-based care delivery.
  • Utilizes a data driven approach including development of key performance indicators with data collection, analysis, and reporting across quality, patient safety, risk management, infection control, outcomes, and patient experience to inform decision-making and prioritize improvement initiatives.
  • Monitor performance metrics (including OBQI outcomes), provide feedback, and drive evidence-based improvements in collaboration with clinical operations leadership.
  • Provide organizational leadership in quality and patient safety through consultation, education, facilitation, and the promotion of process improvement methodologies.
  • Direct compliance program operations, including policy development, investigations, reporting, and collaboration with Dartmouth Health Compliance to ensure adherence to legal and ethical standards.
  • Collaborate with Epic analysts, trainers, managers, and care teams to improve workflows and outcomes.  Lead and support Epic clinical documentation workflows, process improvements, and system optimization initiatives and review Epic updates, assess operational impacts, and implement necessary education and training plans.
  • Support the continued assessment and integration of technology needs.
  • Ensure organizational compliance with all federal, state, and local regulations by maintaining current knowledge, updating policies and procedures, and educating staff and leadership.
  • Supervise and develop departmental staff while fostering strong cross-functional partnerships with leadership, operations, and governance bodies to achieve quality, safety, and compliance objectives.

Qualifications

 

  • Bachelor’s Degree required or each year of approved experience may be substituted for one year of education in nursing or rehabilitation therapies from an accredited college or university.
  • Experience in clinical leadership required
  • Must possess a VT and NH professional license or be license eligible
  • Must have excellent interpersonal, communication and analytical skills.
  • Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.

 

PREFERRED QUALIFICATIONS

 

  • Master’s Degree preferred
  • Interdisciplinary management skills preferred
  • Experience in home-based care
  • Experience with Epic

Required Licensure/Certifications

  • Must possess a VT and NH professional license or be license eligible
