Job Summary:

Under the direction and supervision of the Office of Supportive Housing Assistant Director, the PATH Program Coordinator is responsible to:

• Participate as an Office of Supportive Housing team member in the provision of program monitoring for DBHDD-funded PATH providers statewide.
• Periodically travel to visit program providers in-person around the state and accompany teams during their outreach and service provision in the community.
• Collaborate with internal DBHDD staff and external DBHDD partners.
• Facilitate PATH Provider meetings to encourage consistency of services across the State. This includes presentation of information to large groups.
• Provide technical assistance and training regarding the PATH program, GHVP model, and DBHDD service delivery standards.
• Assist agencies in development and implementation of Quality Improvement Plans.
• Analyze network and provider trends to identify training and technical assistance needs for PATH providers.
• Collaborate with data analyst for data collection, analysis, management and quality assurance functions.
• Assist in the collection, development, and analysis of Key Performance Indicators.
• Represent the individuals we serve, the PATH Program, the Office of Supportive Housing, and DBHDD with professionalism, compassion, and respect.
• Other duties as assigned.

Scheduling and Travel

Scheduling of the 20 hours each week will be flexible.

When visiting a provider and accompanying them in their outreach work, work hours may extend beyond regular 8 am – 5 pm timeframe, as sometimes outreach happens earlier in the day or later into the evening.

Travel time when visiting provider agencies is considered part of your work hours.

Qualifications

Position requires a valid driver's license.

Necessary Skills

• Lived experience of homelessness and/or behavioral health recovery.
• Comfortable with statewide travel. (50%)
• Effective interpersonal skills.
• Experience providing training and/or technical assistance.
• Effective written and oral communication skills.
• Highly organized and attentive to detail.
• Experience with data collection, management, and basic analysis.
• Technological literacy and comfortable learning new systems.

Preferred Qualifications:

• Associate or Bachelor’s degree and at least three years of relevant experience.
• Direct experience working in one of the following: housing programs for individuals experiencing homelessness, outreach programs serving individuals experiencing homelessness, or providing community-based behavioral health services, including any peer coaching or peer supports.
• Knowledge of the homeless system, state and federal housing resources, and adult community mental health services.
• Certified Peer Specialist certification (not required to be hired)
• Experience interacting with the Homeless Management Information System (HMIS) is a plus but not required.
