We are seeking a talented Registered Dietician to join our team at the Daniel Driscoll Neponset Health Center. 


This is an exciting opportunity to join a multidisciplinary team in the delivery of holistic care while achieving key performance measurements in a community setting.  The successful candidate will provide dietetic services for all ages, and across diverse socio-economic and cultural groups in an organization with a commitment to inclusion, innovation and impact! 

Position is part time, 20 hours weekly, days & times flexible

The Dietician home site is the Daniel Driscoll-Neponset Health Center, with both telehealth and in-person visits. The in-person services can be provided out of our Boston, Plymouth or Hyannis health center locations.

Role: 

The Registered Dietician administers optimal nutrition care to health center patients and families through assessing nutrition needs and developing and implementing individualized, evidence-based care plans. The RD will be working with a team including MDs, NPs, RNs, BH, Pharmacy and Social Workers focusing on clinical outcomes and key performance measures in areas including diabetes, weight management, heart disease and more across the continuum of care. 

Responsibilities: 

  • Provides optimal nutrition care to patients by obtaining diet history to determine patient’s past and current eating habits and performing assessment of nutrient needs based on diet history, review of provider referral and/or medical record, and prior discussion with other health care team members, when applicable.
  • Develops and implements nutrition care plans based on approved standards of care to meet assessed needs, including short- and long-term goals; evaluates effectiveness of care plan and counseling in meeting established goals and plans follow up care as needed.
  • Collaborates with interdisciplinary care team and gives integrated nutrition care as needed.
  • Maintains records of nutrition care, documents effectively and communicates pertinent nutrition data though documentation in the medical record in accordance with agency policies.  
  • Maintains ongoing communication with referring provider to ensure continuity of care. 
  • Provides medical nutrition therapy, nutrition education, and food access resources to patients of the Ryan White program.
  • Assists in the development and implementation of nutrition education and referral program for the food pantry and other low-income clients of the agency including quarterly nutrition workshops/cooking demonstrations.
  • Selects and develops culturally appropriate education materials.

 

Requirements:

  • Bachelor’s Degree in Nutrition, Registered Dietitian (RD), Licensed Dietitian Nutritionist (LDN) in the state of Massachusetts required
  • Certified Diabetes Educator (CDE) preferred
  • 1 year of experience in a healthcare setting
  • Ability to communicate in second language Spanish, Vietnamese or Portuguese preferred
  • Self-motivated; ability to work in collaborative, team-based environment
  • Excellent written and verbal communications skills 
  • Familiarity with Electronic Medical Records applications, EPIC preferred.
  • Basic skills in Microsoft applications; specifically, Word, Excel and Outlook

When determining actual base salary and/or rate, several factors may be considered as applicable (e.g. location, years of relevant experience, education, training, and other factors as permissible by law). As such, most offers will not be at the top of the salary range.

*Sign on bonus, half of hire and remainder after 90 days in good standing, former employees of HHSI not eligible.
