The Registered Dietitian (RD) II is responsible for assessing patient needs, developing individualized nutrition plans, and providing education and counseling including Medical Nutrition Therapy, (MNT) to TrueCare pediatric patients . The RD II develops patient-centered individualized nutrition plans based on nutritional assessment, medications, and health status in collaboration with clinical providers and the patient care team. The RD II will work with patients to create S.M.A.R.T goals (specific, measurable, achievable, results-focused, time-bound) that meet their health and wellness goals. When providing nutritional care, Registered Dietitians must support the racial, ethnic, linguistic, and cultural diversity of patients and their families while adhering to evidence-based best practices.

Duties & Responsibilities:

•Assess and screen for nutrition risk(s) using standards established by the Academy of Nutrition and Dietetics.
•Develop, implement, and monitor nutrition care plans per established policies and procedures and provide counseling to patients and family members.
•Provide Medical Nutrition Therapy (MNT) and nutrition education to patients and/or family members using patient-centered counseling skills and motivational interviewing techniques on chronic conditions such as diabetes, heart disease, hyperlipidemias, weight loss including bariatric surgery pre-op and others.
•Provide appropriate age-specific care, with consideration to the individual's educational, cultural, and socioeconomic background. Guide patients and their families to select at least one SMART goal at the end of the appointment.
•Ensure documentation in the Electronic Health Record for all patients is in accordance with TrueCare, HIPAA and other applicable regulatory standards.
•Function as a member of the interdisciplinary team, attend department meetings and serve as a nutrition resource for the healthcare team.
•Develop and/or revise educational materials, classes, training, guidelines, protocols, and other resources to support nutrition services.
•Provide patients with appropriate internal and external referral resources.
•Mentor Health Partners, new dietitians, and dietetic interns, as needed.
•Ensure punctual, regular, and timely attendance in accordance with the TrueCare Employee Handbook; report to work on time and as scheduled and remain at work for the entire duration of each shift; and follow TrueCare policies for notifying the supervisor of and documenting all planned and unplanned absences.

Qualifications:
•Bachelor’s degree in food and nutrition services, nutrition sciences, dietetics, or related field.
•Current Registration with the Commission on Dietetic Registration (CDR)
Current Basic Life Support (BLS) Certification for Healthcare Providers
•A minimum of three (3) years of experience as a dietitian in community nutrition or public health.
•Knowledge of general, therapeutic and community nutrition.
•Knowledge and experience in nutrition education and interventions.
•Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook, Word, Excel and PowerPoint.
•Ensure punctual, regular, and timely attendance in accordance with the TrueCare Employee Handbook; report to work on time and as scheduled and remain at work for the entire duration of each shift; and follow TrueCare policies for notifying the supervisor of and documenting all planned and unplanned absences.

Desired Qualifications:
 
•Master's degree in nutrition, dietetics or related field.
•Knowledge of Maternal and Infant Health, CDPH, CPSP, and gestational diabetes counseling programs.
•Bilingual in English and Spanish.
•Knowledge of clinical competencies based on Nutrition Care Standards of the Academy of Nutrition and Dietetics.

This position pays $43.03 to $ 64.54 on an hourly basis.​
