Liquid Personnel is currently recruiting for an experienced Hospital Social Worker to join our client's busy Hospital Discharge Team in Bury. This is an excellent opportunity for a skilled practitioner with previous hospital social work experience to support adults with complex needs and play a key role in facilitating safe, effective, and timely hospital discharges.
What will your responsibilities be?
- Manage a hospital-based social work caseload.
- Complete assessments and support planning to facilitate safe discharges.
- Work closely with NHS professionals, patients, families, and partner agencies.
- Coordinate care packages and community support to prevent delayed discharges.
- Undertake safeguarding responsibilities and risk assessments where required.
- Maintain accurate and timely case records.
- Provide cross-site cover across multiple hospital locations as required.
- Participate in weekend working on a rota basis.
Qualifications & Experience:
To be successful in this role you must have,
- Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).
- Registered with Social Work England.
- Previous hospital social work experience is essential.
- Experience supporting hospital discharge planning.
- Strong assessment, communication, and multi-agency working skills.
- Ability to work flexibly across hospital sites and weekends.
Compliance Requirements:
- Minimum of 3 years' references.
- Full employment history with any gaps explained.
- Relevant qualifications in line with the role requirements.
