  • Working alongside residents in completing the intake process, which includes program options, required documentation, and develops compliant and accurate treatment plans.
  • Create and maintain strong, positive, and trusting relationships with referring agencies and the surrounding community.
  • Provide accurate program and service information by phone, in person, and via e-mail, as needed.
  • May be requested to assist the Community Relations Liaison.
  • Data entry and reporting.
  • Perform other duties, as required.

  • Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
  • High School Diploma or GED required.
  • 1+ year’s related experience, and/or training or equivalent combination of education and experience.
  • Valid AZ Driver’s License.

Nice to have

  • Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.

Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.



Due to federal contracts the following items are prohibited:  Medical/Recreational marijuana, current parole or probation.



Job Summary: The Admissions Coordinator supports the Admissions Department by managing the daily intake of new clients and effectively communicates with a wide variety of internal and external customers.



Supervisory Responsibilities: 

  • None



Essential Functions:

Qualifications:

  • Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.

Knowledge, Skills and Abilities:

  • Working knowledge of program guidelines and requirements.
  • Working knowledge of 12-step programs.
  • Must be proficient in Microsoft Office Suite and have a working knowledge of Electronic Medical Records systems (EMR).
  • Demonstrated strong organizational and data entry skills.
  • Ability to communicate effectively at multiple levels, both verbal and written.
  • Ability to work independently and as a member of a team.



Education & Experience:

  • High School Diploma or GED required.
  • 1+ year’s related experience, and/or training or equivalent combination of education and experience.



Work Environment:

  • Fast paced behavioral health residential setting.
  • Primarily indoors, however, some outdoor work may be requested.



Physical Demands:

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.



Shift Requirements: 

  • Days, weekends, and holidays.



Travel:

  • Valid AZ Driver’s License.
  • May include travel between sites and other locations.
  • Local and state wide travel when requested or required.
