Adults Social Worker – Learning Disabilities

Liquid Personnel is recruiting an experienced Adults Social Worker – Learning Disabilities for its client’s Community Learning Disability Team based in Havering. This is a full-time agency role offering £30–£32 per hour, supporting adults with learning disabilities to live independently, remain safe, and access the services they need to achieve positive outcomes.

Your Main Responsibilities

• Manage a caseload of adults with learning disabilities, including complex cases where appropriate
• Undertake person-centred assessments, reviews, and support planning
• Complete safeguarding enquiries and risk assessments
• Support safe hospital discharges and prevent avoidable admissions
• Develop and review personalised care and support plans
• Work collaboratively within a multi-disciplinary team alongside health and social care professionals
• Maintain accurate and timely case records and reports
• Contribute to service development and continuous improvement initiatives
• Provide mentoring or supervision support to colleagues where appropriate

What We Are Looking For

To be successful in this role, you must have:

• A recognised Social Work qualification
• Current Social Work England (SWE) registration
• Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
• Good knowledge of the Mental Capacity Act and relevant legislation
• Strong multi-agency partnership working skills
• Excellent communication, organisational, and problem-solving abilities
• A compassionate, proactive, and person-centred approach to practice
• A commitment to delivering high-quality outcomes for adults and their families
