As a Care Partner -Villages, you will play a vital role in supporting older Australians to access the care, services and funding they need to live independently and confidently within our Retirement Living communities.

Your Opportunity 

This is a permanent full-time onsite opportunity based at Warriewood Brook Village on the Northern Beaches, working Monday to Friday from 8:30am to 4:30pm, with minimal travel within the Village. 

As a Care Partner, you will be responsible for guiding residents and their families through their care journey, helping them access government funding, community services and tailored support that enables them to age in place.

Working closely with residents, Village Teams, healthcare professionals and external providers, you will assess needs, coordinate services, develop care plans and ensure residents continue to receive the support they need as circumstances change. 

With empathy, professionalism and a strong customer focus, you will: 

Resident Care Planning & Case Management 

  • Conduct comprehensive assessments of residents' needs, goals and preferences 

  • Develop individualised care plans and budgets that support resident wellbeing and independence 

  • Coordinate My Aged Care assessments and support residents to access appropriate funding and services

What You'll Bring 

You are a compassionate and customer-focused professional who enjoys helping older people navigate the aged care system and access services that improve their quality of life. 

What's Required 

  • Diploma in Community Services, Service Coordination or a related discipline
