Case Manger - Community Integrative Program

Provide direct case management services to all veteran residents with focus on CIP participants. Manage a caseload of up to 21 veteran residents. Facilitate the intake and discharge process as well as client orientation. Complete accurate and timely data entry into the HMIS system. Coordinate care with VA personnel and others on behalf of participants for eligibility documentation, mental and physical health services. Facilitate life skills, informational, and educational classroom presentations and group sessions. Facilitate weekly one on one sessions with veteran residents. Transport veteran residents to appointments, if appropriate.
