DEPARTMENT SUMMARY: St. Charles Home Health Services provides skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work and home health aide services to patients in their homes. Services are provided by interdisciplinary team based on patient’s needs to maximize patient’s outcomes and to prevent hospitalizations.

POSITION OVERVIEW: The Clinical Therapy Manager at St. Charles Health System is responsible for providing oversight of all patient care services and Home Health caregivers. Oversight includes making patient and personnel assignments; coordinating patient care; coordinating referrals; and assuring the development, implementation, and updates of the individualized plan of care. The manager participates in planning, budgeting, staffing, performance, and operation of Home Health in conjunction with the director, as well as performance and annual review of direct reports. This role consults with staff, physicians and administrative members on clinical problems and interpretation of hospital policies to ensure patient needs are met. This position directly manages caregivers in Home Health.         

ESSENTIAL FUNCTIONS AND DUTIES:

Provides leadership and direction to the home health department and effectively manages day-to-day clinical operations.

Performs planning and coordination of workflow. Brings a global perspective to the team.

Manages clinical resources for the agency.

Sets expectation of performance of clinical staff assigned to the department.

Provides for the development of supervisors/caregivers through effective advising, coaching, facilitating, and mentoring.

Ensures consistent and technical excellence and compliance, superior service, and the integration of the healing health care principles.

Accountable for the long-term strategic performance of the department including the identification of business development opportunities and the successful execution of strategic initiatives.

Participates in the Home Health and Hospice leadership after-hours call rotation, providing clinical and operational leadership, consultation, and decision-making to support safe, effective patient care and agency operations during evenings, weekends, holidays, and other designated on-call periods.

Maintains clinical competency. Provides direct home health therapy services within the scope of licensure and professional practice standards as needed to support patient care, staffing, or operational needs.

  

Serves as a real-time resource and assists with clinical expertise for staff with problem-solving on patient care, processes, and family issues. Serves as representative for all customers.

Maintains ongoing communication with staff, department leadership, and others to ensure needs are being met for staff and patients.

Communicates (both written and orally) effectively and maintains professional relationships with caregivers, medical staff, patients, guests and other hospital departments while providing 2-way communication of the SCHS mission and strategic plan.

Oversees department’s practices to meet regulatory agency requirements. Oversees staff documentation for patient care to ensure proficiency in compliance with hospital policies and regulatory agencies

Ensures consistent and technical excellence and compliance, superior service, and the integration of the healing health care principles.

Responsible for budget development, regular monitoring, accountability and meeting all operational targets for all areas within span of control.

Hires, directs, coaches and monitors the performance of all direct reports, to develop and maintain a high-performance team that meets organizational and department goals.

Monitors and ensures all direct reports are current with compliance and safety requirements. Implements and manages all organizational safety directives and goals.

Provides and oversees team’s delivery of customer service in a manner that promotes goodwill, is timely, efficient, and accurate.

Collaborates with teams to review processes and identify/implement opportunities for improvements, applying Lean principles, concepts and tools.

Supports the vision, mission, and values of the organization in all respects.

Supports the Lean principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.

Provides and maintains a safe environment for caregivers, patients, and guests.

Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies, and procedures, supporting the organization’s corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violation of applicable rules, and cooperating fully with all organizational investigations and proceedings.

May perform additional duties of similar complexity within the organization, as required or assigned.

EDUCATION:                

Required: As required by licensure, Bachelor’s degree.

Preferred: Master’s degree.

LICENSURE/CERTIFICATION/REGISTRATION:

Required: Current Oregon Therapy (PT, OT, SLP) license. Valid Oregon driver's license and ability to meet SCHS driving requirements. Ability to travel to business functions/trainings/meetings and all SCHS worksites.

EXPERIENCE:

Required: Five (5) years hospital, skilled nursing facility, and outpatient or rehabilitation experience. Two (2) years’ experience in Home Health. One year of leadership experience.

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

PHYSICAL REQUIREMENTS:

Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.

Frequently (50%): Sitting, standing, walking, lifting 1-10 pounds, keyboard operation.

Occasionally (25%): Bending, climbing stairs, reaching overhead, carrying/pushing or pulling 1-10 pounds, grasping/squeezing.

Rarely (10%):  Stooping/kneeling/crouching, lifting, carrying, pushing or pulling 11-15 pounds, operation of a motor vehicle.

Never (0%):  Climbing ladder/stepstool, lifting/carrying/pushing or pulling 25-50 pounds, ability to hear whispered speech level.
