Job Description:

Job Title: Community Case Expeditor- Behavioral Health- Augusta  

Workplace: Augusta, Ga

Hybrid: 3-5 days per week in office

Travel: 30-50% travel

  

Job Summary:

Under limited supervision by the Regional Services Administrator, the Case Expeditor coordinates and facilitates timely access to appropriate community-based services for individuals transitioning from hospitals and other institutional settings. The position collaborates with individuals, families, hospitals, providers, courts, law enforcement, and community partners to support successful transitions, continuity of care, and sustained community living.

The Case Expeditor serves as a regional resource by providing consultation, technical assistance, education, and service coordination in accordance with DBHDD policies, procedures, and service guidelines.

Key Responsibilities
• Collaborate with Community stakeholders and as directed may represent the region in community meetings
• Coordinate transition and discharge planning among individuals, hospitals, providers, families, and community partners.
• Provide customer service duties including descriptions of services, explanation of the State and Regional system and handling customer complaints
• Facilitate timely access to recommended services and support.
• Coordinate linkages to appropriate services before, during, and after discharge.
• Collaborate with state hospitals and community providers to implement Transition Action Plans and address service needs and/or gaps.
• Monitor and track individuals transitioning from hospitals and other institutional settings and identify barriers to successful community placement.

• Develop solutions with providers and community partners to promote continuity of care and community integration.
• Maintain effective relationships with provider agencies and remain informed of service capacity, individual needs, barriers, and available community resources.
• Provide consultation and technical assistance to providers, hospitals, and other stakeholders.
• Support the development and delivery of training and educational activities related to transition planning and community-based services.
• Promote person-centered planning and meaningful collaboration with individuals, families, providers, and community partners.
• Maintain accurate documentation, tracking systems, reports, and outcome data related to case expediting and transition activities.
• Perform other duties as assigned in support of regional and departmental priorities.
 

Preferred Qualifications
• Master’s degree in a related field and two years of related experience
• Experience working with individuals with significant behavioral health or developmental disability needs.
• Knowledge of DBHDD services, policies, procedures, and community-based service systems.
• Experience collaborating with behavioral health providers, hospitals, courts, law enforcement, and community organizations.
• Experience providing training, technical assistance, consultation, or stakeholder education.
• Strong communication, organizational, problem-solving, and interpersonal skills.

Knowledge, Skills, and Abilities

The successful candidate will demonstrate:
• Proficiency with Microsoft Office (word, excel, access, power point)
• Knowledge of behavioral health and community-based service systems.
• Knowledge of transition planning, discharge processes, and continuity-of-care practices.
• Knowledge of community integration principles.
• Strong advocacy, communication, facilitation, and presentation skills.
• Ability to interpret and communicate policies, procedures, and service requirements.
• Ability to identify service barriers and develop practical solutions.
• Strong critical-thinking, problem-solving, and decision-making skills.
• Strong organizational skills and attention to detail.
• Ability to collect, monitor, analyze, and report case management and transition-related data.
• Ability to work independently and effectively as part of a multidisciplinary team.

This position is a pay grade K on the statewide salary plan.

Employment Information
The position requires frequent collaboration with individuals, families, providers, hospitals, courts, law enforcement, and community stakeholders. Regional travel may be required for hospital and community visits, transition planning meetings, technical assistance, training, and other activities.

This position is located in the Region 2 in Augusta, GA.

Nestled along the banks of the Savannah River, Augusta is the second largest and oldest city in Georgia. Located in the east central section of the state, Augusta is approximately 150 miles east of Atlanta on Interstate 20. Minimum Qualifications:

Bachelor's degree in a related field from an accredited college or university and four (4) years of professional job-related experience which includes one (1) year in a lead worker, team leader, or supervisory role; or two (2) years of experience at the lower level Social Svcs Prgm Consultant 2 (SSP041) or position equivalent.
