What you'll do
- Coordinate discharge planning and ongoing care for patients under psychiatric medication management.
- Schedule follow-up appointments with outpatient psychiatric providers and therapists.
- Support patient access to prescriptions, transportation, insurance coverage, and social services.
- Communicate with pharmacies, families, guardians, and care teams to ensure continuity of care.
- Document case management activities in the Electronic Health Record (EHR) system.
- Participate in interdisciplinary team meetings and contribute to holistic care planning.
