  • Track, assess and document observations regarding client condition, changes in preferences and problems/opportunities, medical orders, family requests and other information pertinent to Care Plan.

  • Works with formal and informal social support systems and families to ensure client’s needs are met.
  • Provide guidance, resources, and information to enable families and clients to make informed medical decisions.
  • Manage Electronic Health records for clients including billing.
  • Coordinate resources for client care.
  • Interact with other professionals on the client’s behalf this may include, physicians, attorneys, fiduciaries, hospice personnel, and community personnel.
  • Create and maintain referral resources, identification, coordination, management and advocacy
  • Additional duties and responsibilities as may be assigned by your supervisor

As a Geriatric Care Manager, you are responsible to provide a holistic, client-centered approach to caring for clients. The Geriatric Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.

Physical Requirements:
● Ability to lift and carry up to 10-15 pounds
● Ability to sit, stand and walk for prolonged periods of time throughout the work day

● Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
