What you'll do
- Provide 1:1 nursing care and treatments according to the patient’s individualized care plan.
- Provide G-tube and PEG tube care, including feedings, medication administration, and site care.
- Perform tracheostomy care and suctioning, and manage ventilator support as ordered.
- Monitor the patient’s condition and promptly report changes to the appropriate healthcare professionals.
- Provide health promotion, counseling, and education to patients and caregivers.
- Administer medications and other prescribed interventions.
- Collaborate with healthcare professionals to coordinate patient care.
- Document patient assessments, care provided, and responses to treatment daily.
- Must be able to lift 50 lbs.
- Must be able to stand and work for extended periods.
- Perform additional responsibilities as required.

- Minimum of 6 months experience in a home care setting.
