  • Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs.
  • Assures timely screenings of patients to allow early interventions on positive findings.
  • Provides accurate billing detail on all services performed at the practice to the biller/coder.
  • Provides effective education to patients that promotes health and wellness.
  • Provides education and information to the practice team that is timely and relevant to the goals of the medical home.
  • Performs pre-visits and patient outreach to assure closure in care gaps.
  • Works quality dashboards to assure care gaps are being addressed.
  • Care documentation is accurate, assuring patient safety.
  • Care concerns from annual wellness visits are addressed timely to provider and/ or office manager.

Hiring Range

$27.30 - $37.58/Hourly

Overview
