What you'll do
- Works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services.
- Focuses on preventive health care as it relates to annual wellness visits and closing care gaps.
- Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs.
- Assures timely screenings of patients to allow early interventions on positive findings.
- Provides accurate billing detail on all services performed at the practice to the biller/coder.
- Provides effective education to patients that promotes health and wellness.
- Provides education and information to the practice team that is timely and relevant to the goals of the medical home.
- Performs pre-visits and patient outreach to assure closure in care gaps.
- Works quality dashboards to assure care gaps are being addressed.
- Care documentation is accurate, assuring patient safety.
- Care concerns from annual wellness visits are addressed timely to provider and/ or office manager.
- Completes charts with accuracy same day of service.
