- Conduct holistic assessments of service users to determine needs for independent living
- Recommend and prescribe appropriate equipment and home adaptations
- Develop and implement care plans to promote independence and safety
- Collaborate with multidisciplinary teams, including social workers and healthcare professionals
- Maintain accurate and timely clinical records

- Degree in Occupational Therapy
- Current HCPC Registration
- Experience in community or independent living settings
- Full UK driving license and access to a vehicle

- Experience with major and minor home adaptations
- Previous experience working within a local authority or social care setting
