What you'll do
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
- Provide care to the most upmost standard

Team 8: Coram, Middle Island, Miller Place, Mount Sinai, Ridge, Rockypoint, Selden, Shoreham, Sound Beach, Upton, Wading River, Yaphank, North Fork , Aquebogue, Baiting Hollow, Calverton, Flanders, Jamesport, Laurel, Mattituck, Northville, & Riverhead.
- 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
- Per visits are required to attend an entire first week of orientation Monday – Friday (8:30AM -4:30PM.)
- Per Visits are required to work 1 shift per pay period.
Job Details
Our Home Care RN’s:

Posted Salary Range
USD $35.00 - USD $135.00 /Hr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
