What you'll do
- Interprets complex provider referrals, evaluates patients, identifies functional limitations, and formulates individualized treatment plans with regular assessment of progress.
- Ensures compliance with system-wide, local, state, and federal regulatory requirements by applying relevant standards and best practices.
- Educates patients and their families about their condition, treatment plan, and strategies for home management.
- Coordinates collaboration with other healthcare professionals as needed to provide comprehensive care.
- Actively participates in departmental meetings and demonstrates engagement in service line rounds, journal clubs, or discharge planning.
- Serves as a preceptor and mentor to new team member as needed.
- Supports educational initiatives within the enterprise, department, and student programming, including but not limited to training opportunities, presentations, student project development, and serving as a secondary Clinical Educator in the absence of the primary Clinical Educator.
- Performs other duties as assigned.

General Summary
Demonstrates advanced clinical skills in evaluating, planning, and implementing individualized Physical Therapy treatment plans. Provides mentorship and contributes to interdisciplinary collaboration and education.

Advanced proficiency in electronic documentation and digital tools (e.g., EMR, Microsoft Office, cloud-based platforms).
