This role will bridge the gap between Home Health & Hospice across 9 Sutter Care at Home locations. Travel to locations is required.Ideal candidate will have experience in one or both clinical areas with preference given to RNs with project experience.

Job Description:
EDUCATION:
- Graduate of an accredited school of nursing
CERTIFICATION & LICENSURE:
- RN- Registered Nurse of California
- BLS- Basic Life Support
TYPICAL EXPERIENCE:
- 5 years recent relevant experience
SKILLS AND KNOWLEDGE:
- Current working knowledge of program area
- Program management experience
- Excellent skills in written and verbal communication to explain sensitive information clearly and professionally to diverse audiences, including non-medical personnel
- Ability to build collaborative relationships with peers and other healthcare providers and systems to achieve programmatic objectives
- Evidence of problem-solving ability
- Effective leadership and communication (interpersonal, written and verbal) skills

Pay Range is $163,259.20 to $228,550.40 / annual salaryThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
