RN - Kidney Transplant Coordinator

What you'll do
- Work with post-transplant patients providing education and coordination of care after their transplant.
- Participate in on call expectations split evenly between all coordinators for days, nights, weekends and holidays.
- Responsible for coordinating a focused and effective effort between the transplant center and its major patient referral sources (e.g. Dialysis Units and Nephrology Clinics) for renal transplantation.

- Works to assure policies, procedures, and all regulatory requirements are met to assure the transplant program stays in compliance with various governing agencies.
- May provide outreach services to regional dialysis and nephrology programs.

- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Minnesota RN license is required within six months of date into position.
- Obtains and subsequently maintains required department specific competencies and certifications.

Come join our Transplant Surgery Team. This team has completed 954 Kidney Transplants since the start of the program in 1989!This Transplant Coordinator will work with our post-transplant patients providing education and coordination of care after their transplant.
Clinic hours are Monday through Friday 8am-5pm with on call expectations. Call is split evenly between all coordinators for days, nights, weekends and holidays.
