The Rural Health Transformation Program Navigator serves as a frontline liaison between community residents and available healthcare and social service resources. Navigators support implementation of Hub-approved community health initiatives and collaborate with local partners to improve community health outcomes.Position Details:
Minimum Qualifications
Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
Strong interpersonal, communication, and problem-solving skills.
Ability to work independently in community settings with limited supervision.

Experience working in rural or underserved communities.
Knowledge of social determinants of health and community-based resources.
