Opening in November at our newly renovated facility at 4740 A Street, the Short-Term Residential program represents an exciting new chapter for Houses of Hope. We are building a team of professionals who want to create a safe, welcoming, trauma-informed environment where individuals experiencing substance use and co-occurring mental health needs can take meaningful steps toward recovery and greater independence.

Houses of Hope is seeking a social worker to join our team. This position supports individuals in the Short-Term Residential program by coordinating services, advocating for their needs, and facilitating treatment and discharge planning in collaboration with the treatment team.

Duties and Responsibilities

• Foster and maintain a safe, welcoming, trauma-informed environment for all individuals served.
• Maintain confidentiality in accordance with federal confidentiality laws, HIPAA regulations, agency policies, and professional therapeutic boundaries.
• Work closely with the treatment team to facilitate treatment goals, service coordination, and discharge planning.
• Provide support, logistical assistance, and advocacy for individuals in service and, when needed, their families.
• Coordinate access to medical, social, educational, housing, transportation, and other appropriate support services, including linkage to more or less intensive community services.
• Identify service and resource gaps during an individual’s stay and assist in addressing those needs in preparation for transition from residential services.
• Develop treatment plans that include recovery goals; meet with persons served to discuss and support progress toward identified goals.
• Maintain clinical records and complete required tasks and documentation accurately and within agency-established timelines.
• Develop and carry out appropriate discharge plans to support a smooth transition from residential treatment and continued access to needed services and resources.
• Conduct post-discharge follow-up with individuals to assess continuity of care, confirm receipt of needed supports, and identify potential barriers.
• Establish and maintain effective working relationships with community agencies, organizations, providers, and individuals.
• Maintain current knowledge and a working database of community resources and service connections.
• Respond to crisis situations and make appropriate interventions.
• Follow and support program and residential rules, safety procedures, and crisis intervention practices.
• Maintain positive staff relationships, work actively as a team member, and communicate regularly with counseling, clinical, and administrative staff.
  

 

Qualifications

• Minimum age 21
• Valid Nebraska driver’s license and proof of insurance; clean DMV record.
  
• CPR/First Aid Certified (will provide training)
• Bachelor’s Degree in social work
• Commitment to trauma-informed, person-centered, and culturally responsive care
• Experience in residential or behavioral health settings, preferred
• Knowledge of substance use, mental health conditions, and 12-step recovery, preferred
• Strong verbal and written communication skills, including skills required for documentation in clinical records
• Ability to establish rapport and effective working relationships with individuals, families, colleagues, community providers, and other stakeholders
