Key Responsibilities
- Support patients and families with emotional and social needs
- Facilitate discharge planning and community referrals
- Collaborate with healthcare professionals

- Maintain professional documentation standards
Requirements
- Recognised Social Work qualification
- CORU registration essential
- Experience in healthcare settings advantageous
- Strong communication and teamwork skills
- Must have own transport and full driving licence
