Social Worker - LD - Liverpool

Liquid Personnel is recruiting an experienced Social Worker – Learning Disabilities Team for its client’s Learning Disabilities Team based in Liverpool. This is a full-time agency role offering £46.78 per hour, supporting adults with learning disabilities to achieve positive outcomes through person-centred assessment, support planning, and safeguarding practice.

What will your responsibilities be?

In this role, you will:

• Undertake strengths-based assessments to identify eligible care and support needs.
• Develop and co-produce person-centred care and support plans with adults, families, and partner agencies.
• Manage a diverse caseload of adults with learning disabilities, ensuring timely intervention and effective outcome-focused practice.
• Complete reviews and monitoring of support plans to ensure needs continue to be met.
• Work within safeguarding procedures to protect adults at risk and promote wellbeing.
• Contribute to hospital discharge planning, transition work, and multi-agency assessments where required.
• Maintain accurate and up-to-date case records in line with legislation, departmental policy, and professional standards.
• Promote independence, choice, control, and social inclusion for adults with care and support needs.
• Represent the department professionally and comply with national legislation, guidance, and local procedures.

Qualifications & Experience:

To be successful in this role you must have:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
