Social Worker - Preparing for Adulthood

What will your responsibilities be?

• Supporting transitions planning - working with young people (14–25) to develop person‑centred transition plans that promote independence, wellbeing and long‑term outcomes.
• Completing Care Act assessments - assessing eligible needs as young people move into adulthood, creating strengths‑based support plans and coordinating appropriate adult services.
• Multi‑agency coordination - collaborating with education, health, SEND teams and families to ensure smooth transitions and consistent support across children’s and adults’ services.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
