Team Leader - Adults and Hospital Discharge

Liquid Personnel is recruiting Qualified Team Leader for its clients Adults and Hospital Discharge Team at Calderdale

What will your responsibilities be?

• Oversee discharge workflow — Lead and coordinate daily hospital discharge activity, ensuring timely, safe, and legally compliant transitions from hospital to community or interim settings.
• Supervise practitioners — Provide oversight, guidance, and decision‑making support to social workers and support staff, including case direction, risk management, and quality assurance.
• Multi‑agency escalation — Liaise with NHS teams, therapy services, and community partners to resolve delays, unblock barriers, and maintain effective multidisciplinary communication.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
