Scope of Position:
The Youth Homeless Demonstration Program (YHDP) Case Manager provides intensive, individualized case management to youth and young adults (ages 18–24) with dependents who are experiencing or at risk of homelessness. The Case Manager partners with participants and collaboratively with community partners to ensure youth receive comprehensive, coordinated services to develop individualized housing stability plans that promote long-term self-sufficiency through stable housing, education, employment, life skills, and community connections while maintaining compliance with HUD, Continuum of Care (CoC), and Youth Homeless Demonstration Program (YHDP) requirements.
