About this role

We are currently recruiting for an experienced Occupational Therapist to join a supportive and well-established Reablement Team within Adult Social Care for its client in Monkton Park.

This is a rewarding opportunity to work within a strengths-based, person-centred service supporting individuals to regain independence following hospital discharge.

What will your responsibilities be?

You will play a key role in helping people recover and rebuild confidence after hospital stays, using your professional expertise to deliver meaningful, outcome-focused support. Working within a compassionate and collaborative team, you will contribute to improving independence, reducing reliance on long-term services, and promoting safe, effective reablement practice.

  • Complete holistic OT assessments within a reablement / community setting
  • Design and implement person-centred reablement plans
  • Support individuals to regain independence and daily living skills
  • Provide expert advice on equipment, safe practice and risk management
  • Act as a trusted advisor within a multidisciplinary team
  • Promote strengths-based practice and positive outcomes
  • Contribute to service improvement and continuous development
  • Work closely with colleagues across health, housing and social care

Qualifications & Experience:

To be successful in this role you must have, 

  • HCPC registered Occupational Therapist
  • Experience in Adult Social Care, community OT or reablement services
  • Strong understanding of strengths-based and person-centred practice
  • Confident completing assessments and managing complex needs
  • Collaborative, adaptable and supportive team player
  • Passionate about improving independence and outcomes
