Palliative Nurse

Assess patients' physical, emotional, and psychosocial needs and develop individualized care plans. Provide nursing care, including medication administration, wound care, catheter care, and symptom management. Monitor and manage pain and other distressing symptoms to improve patient comfort and quality of life. Assist patients with activities of daily living and promote independence wherever possible. Educate patients and family members on disease management, medication use, nutrition, and home care practices. Provide emotional support and counseling to patients and caregivers during illness and end-of-life care. Coordinate with doctors, therapists, social workers, and other healthcare professionals to ensure comprehensive care. Maintain accurate patient records, nursing notes, and care documentation. Respond promptly to changes in patient condition and report concerns to the medical team. Conduct home visits and follow-up assessments as required by the palliative care program. Ensure infection prevention and patient safety standards are maintained at all times. Participate in community awareness programs and palliative care initiatives when required.
