What you'll do
- Manage a complex caseload of service users within a mental health hospital setting
- Facilitate safe, timely, and effective hospital discharges
- Conduct comprehensive assessments and develop care plans
- Collaborate with multidisciplinary teams (MDTs) to ensure continuity of care
- Ensure compliance with statutory requirements and safeguarding protocols
What they're looking for
- Degree in Social Work or equivalent qualification
- Current registration with Social Work England
- Proven experience in mental health social work
- Experience in hospital discharge planning and coordination
Nice to have
- Experience working within a local authority or NHS trust setting
- In-depth knowledge of the Care Act 2014 and Mental Health Act
- Experience in multi-agency working
