About this role

At Epworth Geelong the Allied Health teams are co-located in the one area which encourages interdisciplinary collaborative care, and enables problem solving and learning. We have a small and supportive Allied Health team at Geelong, and strong ties with our Epworth network in Melbourne enabling growth, education and clinical guidance from other Occupational Therapists and specialist roles.

Job Description

Epworth Geelong is seeking to appoint an experienced Grade 1 or 2 Social worker to provide comprehensive social work services on a casual basis.

This role is ideally suited to a clinician with hospital experience, strong discharge planning skills, and a sound understanding of the Victorian aged care sector, including My Aged Care.

You will work in an energetic and cohesive clinical team that strives for excellence in patient care. The social work department offers a high level of clinical support and supervision, as well as a strong commitment to professional development activities and quality projects. Clinical responsibilities will include undertaking comprehensive psychosocial assessments, family work, outpatient work, and discharge planning as well as involvement in student supervision. 

You will make an impact by:

  • Undertaking comprehensive psychosocial assessments and complex discharge planning 
  • Striving for patient and family inclusive practice in all stages of care
  • Engaging positively with all other colleagues to achieve best clinical outcomes
  • Supervising students
  • Demonstrating flexibility and adaptability in a dynamic clinical environment

To be successful in this role you will have:

  • Extensive skills, enthusiasm and commitment to your work and embrace the opportunity to contribute to the positive culture at Epworth HealthCare
  • A minimum of three years of relevant social work experience in a hospital, rehabilitation or other healthcare setting
  • Clinical experience and/or knowledge in the area of brain injury and trauma rehabilitation would be highly regarded
  • Well-developed expertise in assessment, intervention and discharge planning for a complex patient cohort

  • Bachelor of Social Work or Masters of Social Work
  • Eligibility for membership of the Australian Association of Social Worker (AASW)
