- Oversee implementation and day-to-day operations of the CHW pilot, ensuring alignment with program design, goals, and timelines.
- Supervise CHWs, including caseload assignment (up to ~30 members per CHW), geographic alignment, and performance oversight.

The Associate Manager will lead the implementation, oversight, and ongoing management of the Community Health Worker (CHW) Pilot focused on addressing health disparities and improving outcomes among Medicaid members with Type 2 diabetes or other chronic diagnoses.
This role is critical in responding to identified program needs, including high disease prevalence, elevated social determinants of health (SDoH) risk, and disproportionate concentration of high-risk members in targeted counties. The Associate Manager will ensure the CHW program effectively improves member engagement, reduces avoidable utilization (ED visits and hospitalizations), and increases connection to primary care and community-based resources.

The typical pay range for this role is:
$88,374.00 - $190,344.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
