WelbeHealth’s PACE program helps seniors stay in their homes and communities by providing medical care and community-based services. We like to do things a little differently, going the extra mile and making a difference in our participants’ lives.

The Community Health Worker I provides direct center and field-based support to PACE participants by addressing Social Determinants of Health (SDoH) needs and reducing barriers to accessing essential services. This role focuses on assisting participants with connections to community resources, supporting application completion for community programs, and ensuring participants are able to access services that support health, safety, and stability.

This role is different because the Community Health Worker I at WelbeHealth: 

  • Works within an interdisciplinary team (IDT), receive ongoing professional development, and have a direct impact on improving outcomes for older adults through both individual support and community-level solutions
  • Plays a critical role in supporting a growing, mission-driven healthcare organization, ensuring vulnerable seniors feel supported and have access to high-quality care

On the day-to-day, you will:
